Al no doubt felt it was time to finally address an oft-skirted subject on television, that people must depant themselves in order to use the toilet. Some may say it was indecent, but I applaud Al and the animators for their bravery in raising the bar once again.
I never really noticed the guy on the toilet was pantsless. I'm sure they didn't get ultra detailed, though. Family friendly? Well, maybe not, but at least realistic, eh?

I don't know if this been said but in like a surgeon the door al walks in says ICU (I see you). on the wall it says E-Z op. (easy operation not the operation board game).
on amish paradise when he milks the cow he uses a bowl of corn flakes (instead of bucket). and on the fence with all the signs one says sin free zone.

If I'm not mistaken, ICU Stands for Intensive Care Unit. I knew about the last two, but I never noticed the EZ-OP thing.

ICU is intensive care, but I never saw a sign that said EZ Op, and I've been in a lot of hospitals, in a few different states, so I am gonna assume its a local Cali thing, or it was added intentionally.
