Well if you mean why it was called the "
Hernia Rap," that's because Jon first started reciting it on the Monkees tour as a way for Al to change into his costume for..."Living with a Hernia."
Or if you mean why it's referred to as a "Rap," probably because it's not necessarily a song but just a spoken word piece.
Keep in mind of course that it's just a casual name for it. Like the so-called "Fat Jam" at the "end" of concerts, I don't think it was ever really officially titled.

I would think someone who has had a herina would probably not like this song. Cause I'm mean they have lived with a hernia.

Yeah, it would be a little offensive. On the same level as ILOJ being offensive to people who lost on Jeopardy and AP to the Amish.
sarcasm about the 2nd sentence, but the first one is serious.

My dad's had two hernias....I haven't heard any complaints about the song from him when I happen to be listening to it.
