They're all the little polka bits and sound effects and "heys" that are in the polkas that aren't part of one of the other songs. This is oversimplifying it, but it gets the point across -- If there are 12 songs in the polka, Al needs to split the money he makes off the polka 12 ways (1/12 to each songwriter), so by adding a "song" of his own to the polka, it insures sure he gets paid something.

In addition to what Dave said, sometimes the medleys don't go straight from one song to another. There is sometimes small spaces of polka music used to transition from one song to another. Those are covered by the "Ear Booker Polka" and "Way Moby Polka."
BTW, "Ear Booker" and "Way Moby" polkas are really the same thing, just with different names.
If you ever notice though, there are actually two different endings to the polka medleys. There's the one used in Po45, Polka Party, THRP, PYEO, and BH. Then there's a different one (the "shave and a haircut" one) that ends HoP, TAP, Polka Power, and AWBP.
